Self and Professional Referral:
Please complete this referral form for children and young people who would like advice, information and support around their own drug or alcohol use. Please ensure you have consent from the young person before completing the referral to our team.
Affected others referral form
Please complete this referral form for children and young people under the age of 18 who are affected by familial drug or alcohol use. For children 12 years and under we require consent from a parent/guardian with parental responsibility.
